California allows but does not mandate generic substitution. Under California Business and Professions Code § 4073, a pharmacist may select a less expensive generic equivalent unless the prescriber writes "Do Not Substitute" or marks the brand-name box. Pharmacist substitution requires the generic to be an FDA-rated AB-equivalent. Patient consent is required at the point of sale. If the prescriber expressly forbids substitution, the brand must be dispensed.
California's Medicaid pharmacy benefit was carved out of managed-care and centralized as Medi-Cal Rx on January 1, 2022, administered by Magellan Medicaid Administration on behalf of DHCS. The Contract Drugs List functions as the state Preferred Drug List (PDL). Drugs not on the CDL or restricted by clinical criteria require a Treatment Authorization Request (TAR). The TAR process is universal across fee-for-service and most managed-care members for pharmacy claims.

California does not maintain a Medicare-recognized SPAP. Low-income Californians needing drug-cost assistance route through three channels: Medi-Cal Rx (state Medicaid pharmacy benefit), the federal Low-Income Subsidy (LIS / Extra Help) for Medicare Part D enrollees, and manufacturer Patient Assistance Programs. The California Department of Aging operates HICAP (Health Insurance Counseling and Advocacy Program) to help seniors navigate options.
Eligibility: California does not operate a standalone State Pharmaceutical Assistance Program. Medi-Cal Rx provides the primary low-income drug benefit; Medicare Part D Extra Help is the federal LIS layer.
California permits 90-day fills for non-controlled maintenance medications at retail and mail-order pharmacies, subject to plan benefit design. Mail-order pharmacies serving California patients must be licensed by the California State Board of Pharmacy as Non-Resident Pharmacies under Bus. & Prof. Code § 4112. Refill-too-soon rules generally allow refill at 75% of days-supply consumed for non-controlled drugs. Schedule II controls require new written or e-prescription each fill.
California operates the Controlled Substance Utilization Review and Evaluation System (CURES 2.0), administered by the Department of Justice. Health Code § 11165.4 requires prescribers to check CURES before the first prescription of a Schedule II-IV controlled substance to a patient and at least every four months thereafter for ongoing therapy. Pharmacists must report dispensing data within one working day.

California hosts hundreds of 340B-eligible covered entities, federally qualified health centers, Ryan White clinics, disproportionate-share hospitals, and rural referral centers, that purchase outpatient drugs at the 340B ceiling price. Uninsured and under-insured patients of these entities may access deeply discounted drugs at the clinic's contract pharmacy.
