Iowa mandates generic substitution. Under Iowa Code § 155A.32, a pharmacist shall substitute a less expensive generically equivalent drug product unless the prescriber writes "do not substitute," "dispense as written," or signs the appropriate line. The substitute must be FDA AB-rated.
Iowa Medicaid Enterprise administers Iowa Medicaid (IA Health Link) pharmacy benefits and maintains the IA Medicaid PDL. Most members are enrolled in IA Health Link managed-care plans (Amerigroup, Iowa Total Care, Molina), which follow the unified PDL. Non-preferred drugs require prior authorization.
