West Virginia mandates generic substitution. Under W. Va. Code § 30-5-12b, a pharmacist shall substitute a less expensive equivalent drug product unless the prescriber writes "dispense as written" or "brand medically necessary" on the prescription. The substitute must be FDA AB-rated.
West Virginia Bureau for Medical Services maintains the West Virginia Medicaid Preferred Drug List. Most members are enrolled in Mountain Health Trust managed-care plans (Aetna Better Health, Highmark Health Options, The Health Plan, UniCare Health Plan of WV). Non-preferred drugs require prior authorization initiated by the prescriber.

West Virginia does not operate a Medicare-recognized SPAP. Low-income West Virginians use Medicare Part D Extra Help (LIS), West Virginia Bureau for Medical Services (if eligible), manufacturer Patient Assistance Programs, and West Virginia SHIIP for Medicare enrollment counseling.
