Wegovy HD, the 7.2 mg dose of semaglutide, costs $399 a month self-pay on NovoCare Pharmacy, one box of 4 pens. The 2.4 mg pen is $349. Here is what the extra $50 a month covers, and what the trial says it bought.
The answer:
Novo Nordisk announced on 19 March 2026 that Wegovy HD, semaglutide at 7.2 mg once a week, had been approved for adults with obesity. By Novo's description it is for people who have tolerated the 2.4 mg dose for at least 4 weeks and need additional weight reduction, so you reach it from the standard Wegovy pen, not instead of it. The chart sets its price against the rest of the Wegovy range.
| Product | Self-pay, per month | Twelve months | What a month is |
|---|---|---|---|
| Wegovy HD, 7.2 mg pen | $399 | $4,788 | 1 box of 4 pens |
| Wegovy pen, 0.25 to 2.4 mg | $349 | $4,188 | 1 box of 4 pens |
| Wegovy pen, first two fills, 0.25 and 0.5 mg | $199 | n/a | New to the offer, through 31 December 2026 |
| Wegovy pill, 1.5 to 25 mg | $149 to $299 | $1,788 to $3,588 | 1 bottle of 30 tablets |
Annual figures are twelve times the monthly price, a labelled comparison basis and not a prescribed course. Because Wegovy HD is a step above 2.4 mg, a first year on the drug is usually a mix of months at lower doses and months at 7.2 mg, so what you pay over a year depends on your prescriber's plan. Novo Nordisk reports that in STEP UP, 89% of people on 7.2 mg lost at least 5% of their weight, against 38% on placebo, and that the mean starting weight was 248 lb in the 7.2 mg and placebo groups and 257 lb in the 2.4 mg group. The same release reports altered skin sensation (dysesthesia-related events) in 22% of the 7.2 mg group, 6% on 2.4 mg and 0.3% on placebo, and lists nausea, vomiting, constipation, abdominal pain, fatigue, headache, dizziness, hair loss and flatulence among the most common reactions without giving rates for them. These are the maker's own figures from its announcement; a prescriber can weigh them against your health history.
NovoCare's page prices what you pay with no insurance. It does not say what a health plan covers for Wegovy HD or what a coupon does to the price, and we have not read a savings offer for the 7.2 mg dose. Novo Nordisk's announcement says Wegovy HD is available through the channels that carry Wegovy, including retail pharmacies, select telehealth providers, NovoCare Pharmacy and GoodRx, and gives no price of its own, so a pharmacy's cash price for the 7.2 mg pen may differ from NovoCare's. NovoCare also states that eligibility and restrictions apply and that Novo Nordisk may modify or cancel the program at any time, so confirm the price before you order.
How we built this page: we read NovoCare Pharmacy's Wegovy page in a browser on 31 October 2026, choosing the Pen tab and the 7.2 mg dose and declining the site's cookies, and recorded the price and its footnote. We read Novo Nordisk US's press release of 19 March 2026 for the approval, eligibility and trial figures. We did not buy anything, and the percentage-point difference and annual figures are our arithmetic. Prices can change; this page is information about prices, not medical advice.

Yes, by $50 a month on NovoCare's prices: $399 for the 7.2 mg dose against $349 for 0.25, 0.5, 1, 1.7 or 2.4 mg. Over twelve months that is $600.
No. NovoCare's $199 intro price is for patients new to the offer on the 0.25 and 0.5 mg doses, for two monthly fills through 31 December 2026. Its terms then list $349 for the 0.25 to 2.4 mg doses and $399 for Wegovy HD 7.2 mg.
Novo Nordisk says it is approved for adults with obesity, with a reduced calorie diet and more physical activity, who have tolerated the 2.4 mg dose for at least 4 weeks and need additional weight reduction. Whether you qualify is a decision for your prescriber.
In Novo Nordisk's 72-week STEP UP trial of 1,407 adults with obesity and no diabetes, people who stayed on treatment lost on average 20.7% of their body weight on 7.2 mg and 17.5% on 2.4 mg; counting everyone regardless of whether they stayed on treatment, 18.8% and 15.5%. These are trial averages, not a prediction for any one person.
